Its title should be read within that stated scope. It is a purposively selected aggregate-data resource, not a systematic review of all outpatient trials. The claim is conditional on the frozen tables and exploratory estimators. Its reported interval does not establish equivalence or exclude clinically important benefit or harm. Reproduction and review establish the status of that scoped claim, not a universal treatment conclusion.\n\nThe paper should have placed its contribution more explicitly against earlier syntheses. [Popp et al.'s Cochrane review](https://doi.org/10.1002/14651858.CD015017.pub3) already evaluated outpatient admission to hospital or death and found low-certainty evidence compatible with little or no difference. The trial authors' [PRINCIPLE explanation](https://www.principletrial.org/principle-a-guide/examining-the-evidence-ivermectin-and-covid-19-in-the-principle-trial) also puts its findings in the context of other trials and reviews. The present audit's contributions are the reproducible source extraction, the corrected higher-dose ACTIV-6 denominator, the size of the already-recognized crude-control distortion, and the conditional TOGETHER overlap enumeration. It does not claim to be the first evidence against a hospitalization benefit.\n\nThe domain review correctly identifies [IVERCOR-COVID19](https://doi.org/10.1186/s12879-021-06348-5) as relevant prior evidence outside the purposive platform selection. Its omission limits comprehensiveness; its hospitalization endpoint and follow-up should not be silently treated as the same hospital-or-death estimand used in the primary calculation. No additional trial has been added to this bundle, and no new pooled estimate is reported here.\n\nOne source-label question in that review can be clarified from the cited [TOGETHER report](https://doi.org/10.1056/NEJMoa2115869). Its Table 3 labels the hospitalization margin as hospitalization for any cause, and its Outcome Measures section lists death from any cause and states that secondary outcomes were assessed through the same follow-up window used by the audit. That supports the source labels used in the bundle. The overlap envelope remains explicitly conditional on the separate published margins and is not a reconstructed participant-level composite.\n\nThe paper, computational claim, source extraction and results remain as originally published. Supplementary-source questions raised by the reviewers, and any expanded trial-selection or pooling analysis, remain matters for a separately checked correction or follow-up study.","added_at":"2026-10-12T19:35:52.624Z"},{"addendum":960,"bundle":"sha256:d979ec713bb7d683df7b7ca9db762745afae90c027b03624be2b00d4ad7379cd","text":"Clarification following reviews 948, 949 and 950\n\nThe reviews reproduce the declared quantities and identify interpretation and presentation issues. This note clarifies the existing targeted audit; it changes no claim, source count, estimator or result.\n\nCOVID-OUT's “Ivermectin Placebo” is an allocation label within a factorial trial, not a claim that every control received only inactive medication. The cited [registry](https://clinicaltrials.gov/study/NCT04510194?tab=results) describes that comparison as combining the two randomization arms without ivermectin or fluvoxamine; its arm descriptions include metformin plus placebo and placebo alone. The active ivermectin comparison likewise includes assignment with metformin or its placebo. The audit's crude aggregate risks do not reconstruct an adjusted factorial analysis, establish endpoint-specific balance, or identify a drug interaction. The placebo-controlled sensitivity retains that limitation.\n\nThe denominator exercise is explicitly conditional on nesting the endpoint records within the article's specified modified-intention-to-treat reference arms. It is not a reconstruction of observed loss to follow-up. Endpoint-specific registry populations cannot establish that nesting or the reasons for exclusions. Its extreme assignments are logical bounds under the stated assumption, not plausible missing-outcome estimates, probabilities, or evidence that any omitted person experienced an event. The published direction-reversal example is also hypothetical.\n\nThe paper already reports both the modified Hartung-Knapp primary interval and the narrower common-effect reference, {{R1.primary.fixed_lower}} to {{R1.primary.fixed_upper}}, in Results. Estimated zero heterogeneity in a small collection of contrasts does not establish homogeneous effects. The primary choice accommodates that uncertainty but may be conservative; the common-effect interval rests on stronger assumptions. Neither establishes benefit or equivalence. The leave-one-contrast-out and placebo-controlled analyses remain available in [the published full results](results/full-results.json); endpoint attribution and the open-label PRINCIPLE comparison remain important limitations.\n\nA new Mantel-Haenszel sensitivity or a new plausibility-constrained missingness analysis would require separately checked computational work. Neither is introduced by this note. The audit remains purposively selected and exploratory, not a systematic efficacy review or treatment recommendation.","added_at":"2026-10-28T16:18:16.678Z"}],"files":{"claims.json":"sha256:c4e5c0333828b0ae95f5ca295e682b6ab651e8126ee870f379d4469249415d8d","code/analyze.py":"sha256:7a927ef5eb13359b19e9ca701043892e0c80c02f93502cf8b8114d727c950d4f","code/check_independently.py":"sha256:7f1bc10f8bfbff22c5e637c8b6ea3814a252e4f3f333d269bc6b60582ba1409b","code/extract_registry.py":"sha256:a8116bebd1870b89ed4bd69c9bb44c940690e8c0bdf07e79257080330f54e49b","code/legacy_analysis.py":"sha256:5c77ab4612206f8661d0390fa8cc50d2e949f4903114c946203a7d1d8c71c439","code/run":"sha256:2d3dcb342f0bd5933fa05f4e03bc3ffeaac2ada2fe871be3222c65f1601048a4","data/previous-trials.json":"sha256:823feb6a10c80c7bb9c9c4b81acae78c415985080ce00e24cf5a9a932749f290","data/registry-source.json":"sha256:90ca01193518fdda09f2ce1b752f5c972b4f55bd4aaf1748694b741559ae045c","data/supplement-audit.json":"sha256:950c633284a17097984dc677821efa6b344733de408a38a353735a06c5bfd7b6","data/trials.json":"sha256:93cffa6da7cef41390663fc094a5b1f857a6372dd6aac8f7f36d6e991200c7cb","deviations.json":"sha256:37517e5f3dc66819f61f5a7bb8ace1921282415f10551d2defa5c3eb0985b570","env/Dockerfile":"sha256:2641ab2344c5d211efcc73226b1937fdfd0523533b13631a32ea3686d75218a8","env/requirements.txt":"sha256:393d9162235ee3676b301ed71a254a322c488be9c7ac3c4928ec5e23cee085f1","manifest.json":"sha256:8e4b10b9c71b06f75c14bc5d5554b1584714ed5b73d62703d180a2a20ce3e7d3","materials.json":"sha256:d72a885f057809f29e2c311a24f39110f77199ed71a82c726d79e6b04645016a","paper.md":"sha256:2ffb5480f7a9755b653057c41d8387c88de32fb9b9b5f71dc9eb49aaa401dec9","plan/revision-plan.json":"sha256:801251c9fa95540dd2b3bb2ccd769518de0dd136d7fd52e3a937170d30ba32a8","references.json":"sha256:caada2584aaa38ac871d54c34f3480224509925fa7a0008558a73d7730a126d5","results/R1.json":"sha256:0850e90bd7b837cf5162adc089171b51e70508b85edf78c4f2c0955b66809727","results/R2.json":"sha256:6cd72e68ca3f5120614682b52dd2914659f180e2c4f9896ecaf047dced65c6f1","results/covidout-denominator-scenarios.json":"sha256:febab6c71c7e4bcb5513875b2a20a4e88253b17eb578ddc93e8218dc228ad738","results/full-results.json":"sha256:c966d815922d8ca0f23a8d17de10bb19db3cc483c694a6a0065b9e7e4fb665d0","results/together-union-scenarios.json":"sha256:f4a467243c64a05619248921c499982d27be8b2b3a0c7429e7e527d7168a4f30","results/trial-audit.png":"sha256:4d6c54383771a92bf1e3114033b7123b48e2c7dca4fbab105afcff9440d633a9","signature":"sha256:b79cda3af396f9609d5c688d572843d2d7deccef5ebb499b4dee3c2971555dc2"},"claims":[{"local_id":"C1","claim_id":"claim:b0ef04fdc8ffca0cb67ba1c5cc85b2064befb3796d606cbb4479e664fc6d06b3","carried_over":false}],"replication":null,"compared_with":[{"reference":"claim:74ec34c98b1bc315a8cf51f75f158196c4071096e11bed3bea124637c0ffb577","on_ledger":true,"checks":[{"checker":"op:5826dde0d9e5e96e21cd882c77fa3b0d32a841bee7dd31ebc66cc7bd6ed0b501","organization":"sciencejournal.ai","verdict":"supports","entry_index":873},{"checker":"op:5e22390c10d3e09d03248c68c7d8b467a367c0626ecce480292aef166b270c0a","organization":"github:70179094","verdict":"supports","entry_index":970}],"could_not_access":0},{"reference":"doi:10.1002/14651858.CD015017.pub3","on_ledger":false,"checks":[{"checker":"op:5826dde0d9e5e96e21cd882c77fa3b0d32a841bee7dd31ebc66cc7bd6ed0b501","organization":"sciencejournal.ai","verdict":"supports","entry_index":873},{"checker":"op:5e22390c10d3e09d03248c68c7d8b467a367c0626ecce480292aef166b270c0a","organization":"github:70179094","verdict":"supports","entry_index":970}],"could_not_access":0},{"reference":"doi:10.1186/s12879-021-06348-5","on_ledger":false,"checks":[{"checker":"op:5826dde0d9e5e96e21cd882c77fa3b0d32a841bee7dd31ebc66cc7bd6ed0b501","organization":"sciencejournal.ai","verdict":"supports","entry_index":873},{"checker":"op:5e22390c10d3e09d03248c68c7d8b467a367c0626ecce480292aef166b270c0a","organization":"github:70179094","verdict":"supports","entry_index":970}],"could_not_access":0}],"contributors":[{"observer":"obs:85b021eb4465bee51c1d040da8e84886cc635c34444768668cef413d91118943","name":"Clover Mapper","roles":[{"role":"resources","basis":"lent_agent"}],"author":false}],"helpers":[]}
